Title: Advantages and disadvantages of robot-assisted endometrial cancer surgery in comparison with laparoscopic

Abstract

Up to date, in oncogynecology, the most relevant volumes for laparoscopic and robot-assisted surgery are hysterectomy with/without sentinel lymph nodes and hysterectomy with pelvic and paraaortic lymphadenectomy for endometrial cancer. Obesity in patients often accompanies endometrial cancer, which is why we used retroperitoneal access when performing paraaortic lymphadenectomy. We compared the characteristics of 170 operations for the period from 2021 to 2024.Of these, there were 50 RA (robot-assisted) hysterectomies, 50 LS (laparoscopic) hysterectomies, 25 RA retroperitoneal lymphadenectomies and 45 LS.

In the analysis, laparoscopic hysterectomy was performed 43 minutes faster on average than RA (HP-74.2 min.; PA-117 min.) When performing retroperitoneal lymphadenectomy, the average duration of operations in the RA group was 128 minutes longer compared with the results in the laparoscopic group (200 in HP and 328 in RA). It can be noted that the difficulties of operations during retroperitoneal lymphadenectomies in the form of peritoneal perforation in the RA group occurred 3 times more often than in the LS

There was no difference in the number of complications. Interestingly, with RA access during retroperitoneal lymphadenectomy, unlike with LS, it was possible not to do redocing to the right side to perform the paracaval lymphadenectomy. This was achieved due to the articulation capabilities of the instruments. Along with the lack of obvious advantages of RA operations compared to LS, performing operations with both options allows to develop an idea of the stages of operations and improve the interaction of the surgical staff.

Vladimir Alimov has completed his PHD at the age of  33 years from the Russian National Research Medical University, Moscow, Russia. He is a head of oncogynecological department of the Botkin Hospital, the largest hospital in Russia. He has over 30 publications that have been cited over 50 times. He one of the most experienced surgeon in gynecological oncology in Russia. He uses open, laparoscopic and robot-assisted approach in his practice.
