Title: A rare case of disseminated peritoneal leiomyomatosis in a patient with a history of recurrent widespread endometriosis

Abstract

Leiomyomatosis peritonealis disseminata (LPD) is a benign disease characterized by the presence of multiple nodular formations spread over the peritoneum, similar to metastases of a malignant tumor, but having the histological structure of leiomyoma and characterized by prognostically favorable course. The presented clinical case describes the disease of a 47-year-old patient who previously underwent surgical treatment of uterine leiomyoma in combination with endometriosis, followed by repeated surgical operations for widespread recurrent endometriosis. 11 years after the primary operation, the patient was referred to the oncogynecologist with suspected carcinomatous lesions of the peritoneum and pleura. Ultrasound data and cytology of  ascitic and pleural fluid refuted the malignant process, the patient underwent lower-mid laparotomy, multiple biopsy of peritoneal heterotopias, resection of the omentum. According to the results of further morphological and immunohistochemical studies, the resulting phenotype of the tumor corresponds to LPD with foci similar to a smooth muscle tumor with an uncertain malignancy potential.
