Analysis of A Novel Occult Hepatitis B Infection (OBI) Strain Among Women With Breast Cancer

Abstract

Background: Breast cancer constitutes a significant contributor to mortality among women on a global scale. A variety  of  factors,  including  viral  agents,  have  been  implicated  in the etiology of breast cancer. Specifically, the   correlation   between  hepatitis B virus  (HBV) infection  and  breast cancer has  been  documented  in  the  literature. The  objective  of  the  present  study  was  to evaluate the  prevalence of  occult hepatitis B infection (OBI) in the serum samples of women diagnosed with breast cancer.

Methods: The examination of prior research indicated that among 90 cancer patients, 5 (5.55%) were identified with positive HBV markers (HBsAg, anti-HBc, HBV DNA), among them two OBI (2.22%) samples were identified,1 (1.11%) seronegative OBI with lobular carcinoma and one (1.11%) was seropositive with ductal carcinoma, with histological grade II. Both the patients did not receive chemotherapy treatment and HBV vaccination. The analysis of sequencing was carried out for “S” and core regions of HBV genome of the two OBI isolates. The phylogenetic tree was constructed for partial “S” and core regions of the two OBI isolates. The analysis of amino acid of partial “S” and core regions were evaluated for the both OBI isolates by BioEdit version 6.

Results: The examination of partial amino acid sequences pertaining to "a determinant" within both S regions of the OBI strains (OR493437, OR493438) has elucidated their classification as genotype D and subtype ayw2. The analysis of the partial nucleotide sequences of  two isolates core region when compared with the consensus HBV core strain BAJ05148, genotype D from the Taiwan exhibit numerous nucleotide substitutions sequence at positions C1912T, A1919G, A1920G, G1964A, C2009T, C2012T, A2047T, T2053C, T2092A, C2102T, T2107C, T2121G, G2138A, C2139T, T2143C, G2146A, G2152A, T2157C, C2167T, G2177A, T2198C, C2221T, T2227C, A2233G. Furthermore, the investigation of amino acid sequences from the partial core regions of these OBI strains (WLV76436, WLV76437) has revealed that the amino acid sequences of both isolates (WLV76436, WLV76437) display amino acid substitutions at positions  K36G, D51N,Y67H moreover, the isolate strain (0R493437) exhibits an additional amino acid substitution at position V103G, A109I, 115 V>A.The phylogenetic analysis of the S regions has disclosed that both isolates cluster together within genotype D. The findings concerning the phylogenetic tree and the amino acid sequencing of the core regions have revealed that both isolates exhibit a distinct divergence from other HBV genotypes as well as HBV subtypes D1-D12.

Conclusion: The prevalence of OBI among the patients with ductal breast cancer was found to be 2.2%. Both OBI isolates strains showed genotype D, subtype ayw2.  In light of the unique amino acid substitutions identified within the partial core regions of both OBI strains, there exists compelling evidence suggestive of a novel HBV strain associated with OBI; however, this necessitates further scholarly investigation.
