Title: Reconstruction of post-resection defects of the thoracic framework with ultraelastic loop modules made of titanium nickelide in case of tumor pathology

Abstract

In most cases, surgical treatment is required for chest wall lesions. According to review articles of the last 10 years, the average number of complications is more than 30%. The aim of the project is to increase the safety and effectiveness of the reconstruction of post-resection defects of the thoracic framework in patients with an oncological profile by using ultraflexible loop modules made of titanium nickelide. At the Research Institute of Oncology in Tomsk, from 2015 to 2023, 42 operations were carried out with reconstruction of various defects of the chest wall frame with looped modules. Surgery in March 2018 involved broad final resection of 4-6 ribs and the diaphragm. Both patients were reconstructed with individual hyperelastic titanium nickel loop modules. Immediate surgical results were considered good. For all patients, external respiration was evaluated for 7-12 days; based on the results of the Gensler index, the indicators were higher or equal to the pre-operative results. 2 out of 42 cases (4.7%) experienced complications: synaptic infection in the surgical area 7 months after the operation, given that the same infection was registered in the operation area, and 1 case of edge skin necrosis for 10 days due to a violation of blood flow from the flap for a major defect. As of September 2023, data for one case of relapse of leiomyosarcoma 3.4 years after surgery are available. There is no record of instability or implant rejection. The method of reconstructing defects of the thoracic wall with superelastic loop modules made of titanium nickelide showed the possibility not only of restoring the anatomy of the postoperative defect but also of preserving the biomechanics of respiration in the near postoperative period.

Khurshed Khakimov is a final-year PHD student at the Research Institute of Oncology, Tomsk NRMCR « National Research Medical Center RAS», Tomsk, Russia. He is an Oncologist in the General Oncology Department at the Research Institute Tomsk. He has published 5 scientific papers in domestic and foreign publications.
