Primary extragonadal choriocarcinoma of jejunum – life threatening & clinical paradox
Abstract
Choriocarcinoma is a highly malignant, aggressive germ cell tumor arising from trophoblastic cells of gonads, commonly presenting as gestational trophoblastic tumor in females. However, extragonadal lesions are rare, involving lung, kidney, central nervous system, liver, and gastrointestinal tract. Gastrointestinal choriocarcinoma commonly affects stomach, rectum and intestine. Jejunum is extremely rare.
We report a case of primary choriocarcinoma of jejunum, presenting with pain abdomen, severe anaemia and melena, clinically indicating ectopic pregnancy. Serum beta human chorionic gonadotropin (βHCG) level was elevated while ultrasonography revealed bilateral simple ovarian cyst and MRI showed cystic follicles in ovaries,? adenomyosis in uterus and lesion in Rt. lobe of liver. Laparotomy revealed a granulomatous mass in jejunum, 25 cm away from duodenojejunal junction. The resected specimen revealed choriocarcinoma of jejunum on histopathological evaluation. As per our review of English literature so far, only around thirty cases of primary choriocarcinoma of jejunum has been documented. Thus, serum β-HCG estimation is an excellent marker for early diagnosis and surveillance of choriocarcinoma with poor prognosis.

Biography
Shahabuddin is a Consultant Pathologist and Quality Manager at Sonoscan Healthcare Private Limited, Malda, India, with over 23 years of experience in pathology and medical laboratory quality management. He completed his MBBS from Dr. B.R. Ambedkar Medical College, Bangalore, in 1995 and MD in Pathology from Kasturba Medical College, Mangalore, in 2000. He previously served as an Assistant Professor in the Department of Pathology at SMIMS, Gangtok. He is a Member of the Indian Association of Cytologists (MIAC) and has received specialized training in medical laboratory quality systems and internal auditing in accordance with ISO 15189 standards. Shahabuddin has authored eight papers in national and international indexed medical journals and also serves as a peer reviewer for indexed medical journals, contributing to advancements in pathology, cytology, and laboratory quality management.
